So many people do not have dental insurance, and even when they do, their co-pays are high. UPFH has donors and grants to keep cotst down! Because of these donations, we have a unique process for getting an appointment.

To make an appointment, please come into our office with 1) your ID , 2) your insurance cards, if applicable, & 3) your Income Verification Documents  if you are applying for assistance. Click here to see which documents verify income.

You must pay a $25 down payment in order to make an appointment. This payment applies to your bill, leaving you less to pay later and helps us insure that care is going to those who are available.
