AT Utah Partners For Health

Who We Are

Utah Partners for Health is a nonprofit, community-based healthcare organization dedicated to serving uninsured and underserved populations across Salt Lake and Tooele counties. What began as a small partnership has grown into a network of medical, dental, behavioral health, pharmacy, mobile, and vision services — all rooted in a patient-centered medical home model.

Our growth has always been guided by community needs, strong partnerships, and a belief that access to care should never depend on income, insurance status, or circumstance.

Our Mission

Utah Partners for Health exists to be everyone’s first choice for medical care by providing compassionate, comprehensive, high-quality services that empower underserved individuals, families, and communities. Through a patient-centered medical home approach, we are committed to improving health outcomes, removing barriers to care, and strengthening the communities we serve.

Our History

2002-2003
Founded to Serve
Utah Partners for Health was founded to address the growing number of uninsured individuals. The organization received 501(c)(3) status and served its first patients through early community partnerships.

2007-2013
Expanding Access Through Mobile Care
UPFH launched its Mobile Clinic and Prenatal Clinic to expand access in underserved neighborhoods. The Mobile Clinic grew rapidly and continues to serve thousands across multiple counties.

2013–2014
First Permanent Clinic
UPFH opened its first permanent clinic in Midvale through an HRSA New Access Point Grant, expanding services and integrating the Mobile Clinic into its core operations.

2015–2016
Broadening Essential Services
The organization added mobile vision services, purchased its first administrative building, and opened behavioral health and dental clinics to meet growing community needs.

2017–2018
Meeting Growing Demand
Dental and behavioral health services expanded to full capacity, including added providers, extended hours, and enhanced case management for patients with complex needs.

New Clinics & Treatment Programs
UPFH introduced a new Mobile Medical Trailer, launched medication-assisted treatment services, and opened the UPFH Family Clinic in West Jordan.

Integrated Care & Growth
New leadership supported major service expansion, including integrated behavioral health, pharmacy services, mobile MAT, patient navigation, and major clinic renovations.

2022–Present
Continuing to Move Forward
UPFH continues to grow through federal funding, partnerships, and community-driven planning — expanding dental, vision, counseling, and primary care services.
