Understanding the Sliding Fee Discount
UPFH is not a free clinic, but we offer some of the most affordable care in the valley through our Sliding Fee Discount Program. By providing income verification once a year—even if you have insurance—we can help reduce your office visit payments and make high-quality care more accessible.
Income verification simply means showing proof of what you earn. Your discount is based on your household income and family size, and some discount levels may also require proof of address. It’s a quick, simple process that ensures you receive the lowest cost for the care you need.
Family Income
How to Determine Your Household Income
Check if you or your spouse/partner earn income.
- If neither of you earn income, please see a Front Desk Manager.
- If yes, your household income = your income + spouse/partner’s income.
Are you a student or still dependent on your parents?
- If no and you don’t earn income, please see a Front Desk Manager.
- If no and you or your partner earn income, calculate income: your household income = your income + spouse/partner’s income.
- If yes, do you or your spouse/partner pay more than 50% of your expenses? OR are you a female receiving reproductive health services?
- If yes, your household income is your income plus your parents’ income.
- If no, your household income stays as your income plus your spouse/partner’s income.
Family Size
Calculating Your Family Size
Please add up the total number of people who rely on your income to determine your family size. This includes yourself, your spouse, and any children who depend on your earnings. You should also count any other individuals who rely on your financial support, even if they don’t live with you or are not related to you. For example, a nephew living in your home or a parent living abroad who depends on your income would both be included. The final number is your family size.

Don't Forget Your Docs!
To quality for discounts, all patients have to show 1) ID (driver’s license or other), 2) insurance card (if applicable), and 3) proof of income (choose from below):
Proof of income options:
- 1040 form, previous year
- Two current paycheck stubs
- Letter from employer stating earnings. Letter must be 1) on employer letterhead and 2) be accompanied by a business card
- A recent document from unemployment, disability, pension, retirement, annuity, military family allotment, or social security
- Letter from a Pastor, Bishop, or shelter confirming hardship or unemployment
- Copy of a court order settlement like alimony
- the last 3 months of bank statements
Good faith Estimate
Understand Your Good Faith Estimate
When you visit UPFH, you can request a Good Faith Estimate of your expected visit costs. Keep in mind, this is only an estimate. Additional services, such as an x-ray during your appointment, may result in extra charges.
You can ask for pricing before your appointment or during your visit before any procedure is performed.
