Tony Gregory, Medical Director

Tony Gregory is a compassionate and experienced Physician Assistant with over 13 years of experience in Family Medicine, complemented by a background in Emergency Medicine and Urgent Care. Fluent in Portuguese and proficient in medical Spanish, Tony is dedicated to providing culturally sensitive care and reducing barriers to healthcare access for diverse communities. In addition to practicing the full spectrum of Family Medicine, Tony has a special interest in Addiction Medicine, helping individuals and families navigate recovery with dignity and support. Outside of medicine, Tony enjoys spending time outdoors and cherishing moments with family. Passionate about patient centered care, Tony strives to empower patients through education, connection, and comprehensive care within the medical home model.

Sarah Woolsey, Medical

Sarah Woolsey is a board certified family physician with 25 years experience in primary care. She is honored to to practice part-time at Utah Partners for Health. She graduated from the University of Illinois College of Medicine and University of Utah Family Medicine Residency.

Tania Oliveira, Medical

I am originally from Brazil and love reading, traveling, dancing, good food and gathering with family and friends. I have been at Utah Partners for Health since 2017, first as a nurse, then as a provider. I love the people and community we serve.

Nadia Xibille, Medical

I am a bilingual Physician Associate, fluent in both Spanish and English, with a heart for helping others live healthier, happier lives. I am originally from Mexico, I earned my Bachelor’s degree in Nutritional Sciences from the University of Arizona and my Master of Physician Assistant Studies from Touro University Nevada.

Since 2012, I’ve had the privilege of working in family medicine, where I love building lasting relationships with my patients. I am passionate about empowering individuals to better understand and manage their medical conditions, offering support, education, and personalized care every step of the way.

Brenda Espitia, Medical

I am a Latina Physician Assistant who graduated from the University of Utah Physician Assistant Program in 2021. I began my clinical career in community health in Orange County, California, serving diverse patient populations. I am dedicated to advancing community health through high-quality, patient-centered care. I am bilingual in English and Spanish, and value providing culturally responsive care to meet the needs of my patients. I am pleased to continue my professional journey with Utah Partners for Health.

Philip Swink, Medical

Since 2017, Phil has been practicing as a Physician Assistant. He studied Biology and Spanish at the University of Utah before completing his Master’s Degree in Physician Assistant Studies at Indiana State University. Prior to UPFH, he’s practiced Geriatric & Palliative medicine as well as Wound Care.

An English and Spanish speaker, he enjoys serving the patients of the greater Salt Lake community and continually learning. Outside of work, he enjoys exercising, outdoor sports, board gaming, and best of all, spending time with his wife and two children.

I am a Latina Physician Assistant who graduated from the University of Utah Physician Assistant Program in 2021. Jack Wang, Vision

Jack Wang attended The University of Texas at Austin for his undergraduate studies. He then graduated from the University of Houston College of Optometry and has been practicing since 2005. He is mainly focused on primary eye care. Outside of work, he enjoys spending time with his family and 2 dogs. Other interests include keeping up with current events, skiing, and exploring new recipes.

Heidi Schiller, Pharmacist

Heidi Schiller, Pharmacy Director Heidi Schiller was born and raised in Utah. She has a passion for providing the best care and pricing for our underserved population in West Jordan and the surrounding areas. She graduated from Roseman University with her Doctorate in Pharmacy and Masers of Business Administration in 2021. Upon graduation, she began her career by opening Utah Partners for Health pharmacy.

Since 2021 she has helped open three pharmacies in Utah and is dedicated to providing exceptional pharmaceutical care throughout the Salt Lake Area. She dedicates her time at Utah Partners for Health in helping patients better understand their medications and side effects and is particularly passionate about diabetic care. Heidi believes that the 340B medication program plays a vital role in providing affordable medications to the underserved population. Heidi enjoys hiking, camping, snowboarding and spending time with her family (daughter Raven, son Xander, and husband Max).

Patricia Esparza, Pharmacist

With over 15 years of experience serving the communities of Utah, Patricia Esparza is a dedicated and compassionate pharmacist known for her personalized care and deep commitment to patient education. Fluent in both English and Spanish, she bridges language gaps to ensure that every patient fully understands their medication regimen and feels empowered in their healthcare journey.

Throughout her career, Patricia has built lasting relationships with patients and providers alike, emphasizing clear communication, attention to detail, and evidence-based practice. Whether in a retail or community setting, she brings a calm, reassuring presence and a deep understanding of the unique healthcare needs across Utah’s diverse population. Outside the pharmacy, she loves exploring the red rock landscapes of Southern Utah with her husband. From hiking in Moab to discovering lesser-known desert trails, the outdoors provide a perfect balance to her professional life.

Justin Zamora, Behavioral Health Director

a licensed psychologist and health service provider in psychology (HSPP). He is the Director of Behavioral Health at Utah Partners for Health, a Federally Qualified Health Center (FQHC) with clinics in Midvale and West Jordan, where he provides bilingual (Spanish and English) psychotherapy to adolescents, adults, and couples. 

He holds a master’s degree in marriage and family therapy and a doctoral degree in counseling psychology. Dr. Zamora has completed extensive post graduate psychoanalytic training at multiple institutes, including: William Alanson White Institute (NYC), Stephen Mitchell Relational Studies Center (NYC), Training and Research Institute in Intersubjective Self Psychology Foundation (NYC), Institute for the Psychoanalytic Study of Subjectivity (NYC), Center for Modern Psychoanalytic Studies (NYC), Psychoanalytic Psychotherapy Studies Center (NYC), Institute for Modern Psychoanalysis of Philadelphia, and the Institute of Contemporary Psychotherapy and Psychoanalysis (Washington, DC). He is a 2025 Division Scholar for the Society for Psychoanalysis and Psychoanalytic Psychology (SPPP; Division 39 of APA) and a recipient of the 2025 Early Career Professional Award from the International Association for Psychoanalytic Self Psychology (IAPSP). 

Alexis Anthon, Behavioral Health

My name is Alexis and I am an associate mental health counselor that works with adults who are navigating the challenges of anxiety, depression, and trauma-related concerns. As a trained EMDR (Eye Movement Desensitization and Reprocessing) therapist, I utilize this evidence-based modality to help clients process and heal from past trauma, allowing them to break free from the negative thought patterns and emotional distress that may hold them back. My goal is to create a safe, authentic, and supportive space for my clients to explore their emotions, gain insights into their struggles, and develop practical tools for lasting change.

Xochitl Juarez, Behavioral Health

Xochitl graduated with a Master of Social Work from the University of Utah. She’s been in the Social Work field for 10 years and has experience working with a range of diverse backgrounds including youth in foster care, first-generation individuals, and Spanish speaking populations. Xochitl believes therapy can feel like “cafecito con pan”—warm, grounding, and rooted in connection. She’s passionate about building culturally and linguistically responsive services that meet people where they are, one platica at a time, using a humanistic, experiential, and intersectional feminist lens.

She specializes in intergenerational trauma, cultural dynamics, the immigrant experience, and parenting dynamics. She believes therapy is a sacred space to explore your story, reconnect with your inner strengths, and create healing that resonates across generations. Xochitl is a proud mother of three boys and enjoys creating memories with them by spending quality time exploring new activities to do together. She enjoys a belly hurting laugh, eating a really good meal, and being around the people that make her feel like home.

Melanie Montelongo, Behavioral Health

My name is Melanie Montelongo-Bernardino, and I am a bilingual therapist who works with children and adolescents. I earned my Master’s of Social Work degree from the University of Utah. In my work, I use Play Therapy, Sand Tray, Expressive Therapy, TF-CBT, Mindfulness, and Strength-Based approaches to support children in building confidence, resilience, and healthy emotional skills. As a first-generation Latina, I bring a deep respect for lived experiences, personal strengths, and cultural values into my work.

I take a trauma-informed approach that centers safety and compassion, while recognizing how identity, culture, and life experiences shape the healing process. I am especially passionate about serving the Latino community in Utah to reduce the mental health stigma and increase access to culturally responsive care. Outside of work, I enjoy dancing cumbia, reading, traveling, and spending time with loved ones.

Nancy Raymond, MAT/Therapy

Nancy Raymond, DNP, PMHNP-BC, FNP-C Nancy Raymond is a dedicated mental health provider with over five years of experience supporting individuals on their journey to improved mental well-being. She is passionate about helping her patients achieve their mental health goals through compassionate, patient-centered care. Nancy recently earned her Doctor of Nursing Practice (DNP) degree from Chamberlain University, further advancing her clinical expertise. She is proud to be part of the team at Utah Partners for Health, where she works alongside a close-knit, supportive group of colleagues who feel like family.
